McSteamy joined SGH much to the dismay of Addison and joy of Karev. McSteamy starts earning his paycheck right away when a patient lights a cigarette in his room while he's got a cannula which is hooked up to an oxygen tank. His face catches on fire. The patient makes it through the surgery okay, but his wife is worried about his face -- he's a car salesman. McSteamy tries to explain that his face may not ever go back to what it was and falters in trying to comfort the patient and his wife. Bailey (whom McSteamy was dismissive to earlier) saves him by refocusing the patient and wife on what's important. People who love and care for you.
Addison lets Karev work with McSteamy and ends up being ordered to fetch coffee. He starts to get the idea that McSteamy -- kind of a jerk. By the end Addison lets him off the baby squad and tells him that he's free to pursue plastics with McSteamy, but that he'll miss her as a mentor.
Addison has patient who is unwilling to have a C-section even though the baby seems unwilling to be delivered naturally. Bailey observes that McSteamy has Addison flustered and second-guessing herself and that she needs to keep her knee together around him. Karev finds Addison when the baby goes into distress, and Addison gathers her wits and tells her patient that what she wants (natural birth) is no longer and option and that they're operating. Mom and baby make it through
Meredith has to have an appendectomy and spends the episode dopey from the morphine. It causes her to tell the truth and wipe her nose constantly. She tells McVet, McDreamy, and George (all in the same room) that they're all good kissers. She asks Addison how she knew Derek was the one and Addison answers (only because she knows that Meredith will have no recollection of the conversation) that she knew he would never hurt her. McVet and McDreamy compete by her bedside and have a nice moment with each other. McVet shows what a good guy he is as he and McDreamy bond over fishing. McDreamy asks the Chief why he walked away from Ellis, and the Chief responds that he knew he couldn't make her happy and had to walk away to be the better man. McDreamy then tells Meredith (post-surgery, sober, and mortified) that he's walking away and that McVet is good for her which is when Meredith realizes that she wants him. She lets McVet down gently, and thus ends Chris O'Donnell's guest stint on GA.
Burke gets cleared for surgery although he's doubtful and hiding residual hand tremors from Christina who is over-eager to get "Preston Burke" back. He's practicing on a corpse when Christina confronts him over his doubts and he confesses the tremors. He can't finish his surgeries with the weakness and Christina says she'll help him by holding the valve and that no one has to know.
Callie and George kind of make up when she invites him back to her hotel room that night, but George has to cancel last minute because Izzie needs him. Callie then realizes that George still doesn't know what he wants when it comes to her and that she doesn't come first, so they break up again. Callie runs into McSteamy at the bar and invites him back to her hotel.
Izzie meets with Denny's dad (Denny, Sr.) who questions her motives when it came to his son. He was suspicious that she just wanted Denny's money. Izzie defends Denny, saying that she doesn't understand how his parents abandoned him and were never there for any of his surgeries. Denny Sr. tells Izzie that they thought Denny was dead because he left them to protect them from seeing their son die. He now realizes that Izzie loved Denny and leaves her a phone number and code and envelope to call and listen to the voicemail Denny left his parents.
Voice over at the end was Denny's voicemail about how he was sorry that he ran away. He said that it was a mistake and that he wanted them to come to SGH right away because he met the woman of his life. While Izzie is listening to the message, George looks in the envelope, and Denny's left her an $8.7 million check.
